BEFORE PROCEEDING, PLEASE READ ALL INSTRUCTIONS CAREFULLY

STEP 1
IF YOU DO NOT HAVE THE LATEST VERSION OF READER INSTALLED ON YOUR COMPUTER:
Simply click on the "Get Acrobat Reader" icon. Follow the easy 1-2-3 steps at the Adobe Acrobat web page for upgrading. Be sure to select the correct operating system that is on your computer. PC users will have the upgrade install by itself after the download is complete. Mac users will need to install the upgrade themselves by double clicking the file that downloaded from the Acrobat site. When the downloading is complete, go to step 2.
STEP 2
IF YOU ALREADY HAVE THE LATEST VERSION OF THE ACROBAT READER:
Click on the order form to begin ordering.

a.   Fill out the Order Form in its entirety.
b.   Print out the completed Order Form.
c.   Fax the printed Order Form to us at: 800-875-7700 or 269-327-8362.
d.   If you have artwork, fonts, or a layout that needs to be attached, click here.
e.   When filling out the email, in the subject window, refer the mainline on the faxed Order Form.
Please Include all dealer information, mainline on the order itself, and a summary of what is being ordered.
